Lymphoedema Education Solutions COVID 19 Course Guidelines and Health Declaration

Lymphoedema Education Solutions (LES) takes seriously the well being and health of each individual that is associated with its courses. It is guided by the health directive of the Victorian state government.

COVID 19 Course Guidelines

Lymphoedema Education Solutions will provide a level 2 face mask to course participants, tutors, invited guests for all sessions. LES will also be guided by the Victorian Health Department for any other changes.

Where possible, maintain a 1.5 meter distance from others at all times (exempted during practical
sessions). And be mindful of this when using public amenities.

Please bring your own pen and a water bottle clearly named. Catering will be individually plated and cutlery individually bagged.

Named tubs will be provided that will contain the linen and other equipment that you will individually use during the duration of the course.

All plinths and shared equipment ( such as measuring boards) will be wiped down with Clinell Universal Disinfectant wipes between individuals using them.

The following health declaration is to be completed by course participants, tutors, lecturers, exhibitors and visitors before attending courses. To reduce unnecessary burden on you, we ask that you complete this form once – but remind you that by completing this declaration ¬you are agreeing to abide by the health, hygiene and physical distancing measures required to contain the spread of COVID-19 in our community – every time you attend the course.
One of the most important measures to keep us all safe is to stay home if unwell and do not attend the course. If you are unwell with a new illness, and are experiencing any of the following symptoms: fever, chills or sweats, cough, sore throat, shortness of breath, runny nose, loss of sense of smell or feel generally unwell, please stay home.

You will be asked to sign a form each day you attend the course that declares that you are well and have none of the symptoms mentioned above.

I agree that every day I intend to come to the course • I will review my answers to the three health check questions, and NOT come to the course if I answer yes to any of them • I will decide to stay home if I am feeling unwell with fever, chill or sweats, cough, sore throat, shortness of breath, runny nose, loss of sense of smell or feel generally unwell • If using public transport to come to the course I will plan my trip and consult advice produced by Public Transport Victoria (PTV) and abide by the physical distancing and hygiene measures suggested by PTV.
I agree that whilst at the course I will wear a face mask during sessions that are advised by Maree O’Connor and that they will be provided by the course organiser. • Maintain hand hygiene, washing/sanitizing my hands regularly • Use good cough and sneeze hygiene (into a tissue with safe tissue disposal and washing hands or into my elbow) •Only spend the necessary time at the course.
I agree that if I become unwell whilst at the course, or think I might have COVID-19 related symptoms I will:1. Notify Maree O’Connor, course convenor, 2. leave the course immediately, 3. Get tested for COVID-19 if I have the symptoms associated with it.
